Singapore
Ministry of Education wanted to capture information about
teaching standards from both students and teachers.
Kelicon provided them with two questionnaires. One to
be used by the students and one by the teachers. The

The first thing to decide is if you want a welcome page. This page can be used to
introduce your respondents to the questionnaire and what you hope to achieve from it.
You can also use this page to collect information such as name, email address or other
information relevant to your questionnaire.
Next you need to decide on the pages you want displayed. The first page might be asking
for information to allow you to analyse the responses. Below is an example of
standard, single selection, option boxes.
You may then want to ask questions where multiple answers are allowed.
You can also ask questions where you want a qualitative response using slider bars.
